As a first point of contact into Mildmay Mission Hospital, the post is one of the key faces of the organisation and the expectation is to provide excellent customer care to referrers, commissioners, patients and their carers.
To coordinate the patient admission pathway
To lead in developing and implementing strategies agreed with the Chief Executive to establish and effectively manage relationships with non-contract Clinical Commissioning Groups (CCG), HIV Clinical Nurse Specialists (CNS), Homeless pathway teams and acute Hospital Units.[SR1]
To facilitate improved communication processes for Extra Contractual Referrals (ECRs) referrals within non-contract areas.
To work autonomously to obtain the clinical history of patients with complex physical, psychological, mental health and social needs as defined by local policy/national guidelines across many different clinical settings.
Mildmay Mission Hospital is at the forefront of care and development of services in relation to complex HIV related conditions for patients across the UK and step-down care for Homeless patients across London.
The Care Quality Commission rated us outstanding. Rehabilitation is our passion with a focus on Neuro cognitive impairment. We use a multidisciplinary (MDT) team approach to provide a range of holistic services to our patients and clients.
